Application Form

A Common application form is applicable to all such academic courses at undergraduate and post graduate levels on merit basis. The Prospectus and application form can be obtained on cash payment of Rs. 1000/- from the Office at :

• (1). City Office – F2, Sangam Place, Civil Lines, Prayagraj (U.P.)-211001
• (2). Campus Mavaiya, Near Sarawati Hightech City, Naini, Prayagraj (U.P.)-211010

Important enclosures with the application form: Students are required to submit the following documents along with the application form.

•  Self attested copies of high school / Intermediate / Graduation Mark Sheet and Certificate (in two sets).
•  Character certificate of the last attended Institution.
•  Transfer/migration certificate in original.
•  Proof of weight age claimed (Caste Certificate) or any other certificate required in reference to application form (in two sets).
•  Five copies of passport size photographs, one affixed on the application form.
•  Domicile Certificate (in two sets).
•  Income Certificate (in case scholarship form filled) in two sets.
•  AadharCard & PAN Card (for Account opening)
•  Envelop each with 5 Rs. Stamp affixed.

Note : 

1. (1)For LL.B. & B.A.LL.B. Students taking admission in Prayag Vidhi Mahavidyalaya should also fill the online admission form of prof. Rajendra Singh (Rajju Bhaiya) University.
2. (2)For B-Pharma, Students taking admission in R.K. Collage of Pharmacy should also fill online admission form of CUET.
3. (3)For D-Pharma, Students taking admission in R.K. College of Pharmacy should also fill online admission form of JEECUP.

Scholarship Details

•  Minimum 60% are required in the eligible category for scholarship.
•  Income and Caste certificate must be uploaded on the official scholarship website (www.scholarship.up.gov.in)
•  Scholarship provided under government schemes, the institute is not responsible for any delays, rejections, or conditions related to government policy.
